 Safe, stable, and affordable housing plays an important role in healthy aging and overall wellbeing. At Empowering Support for Wellness & Aging (ESWA), our Housing Supports services help older adults access housing-related resources and supportive programs that promote independence, community connection, and quality of life. Whether someone is seeking a supportive living environment, assistance navigating housing options, or opportunities to remain engaged and connected, ESWA is here to help. 

Housing needs can change over time, and finding the right resources is not always easy. ESWA works with older adults, younger individuals with disabilities, family members, caregivers, housing partners, and community organizations to connect individuals with services and support that help them remain safe and independent. Our Housing Supports services are designed to address both practical needs and overall wellbeing, recognizing that successful aging involves more than housing alone. 

Through our Housing Supports programs, we help older adults and younger individuals with disabilities access housing-related services, community resources, wellness opportunities, and supportive environments that encourage independence and social connection. The programs below provide additional information about the housing and wellness services available through ESWA.

Independent Living with Supportive Services

Congregate Housing

The Congregate Housing program is a shared living environment designed to integrate the housing and services needed by older adults and younger individuals with disabilities.

ESWA’s Congregate Housing Program offers older adults and younger individuals with disabilities an affordable, supportive living environment that promotes independence and community. Residents enjoy the privacy of their own living space while sharing common areas such as kitchens, dining rooms, lounges, and other community spaces. 

Designed for older adults and younger individuals with disabilities who can live independently but benefit from a supportive setting, the program combines privacy with opportunities for social connection, engagement, and access to services. A dedicated Congregate Housing Coordinator helps residents connect with resources, navigate challenges, and maintain their wellbeing. 

Congregate Housing is neither a group home nor an assisted living setting and does not provide 24-hour care or supervision. The coordinator does not provide residents with hands-on medical or nursing care or assistance with activities of daily living (ADLs) or instrumental activities of daily living (IADLs).

By balancing independent living with shared spaces and supportive services, Congregate Housing helps residents remain active, connected, and confident in their community.

Supporting Healthy Aging Through Connection and Engagement

ESWA partners with Colony Retirement Homes to provide wellness-focused programs and activities that support healthy aging and enhance quality of life for their residents. These programs are designed to promote physical well-being, social connection, education, and engagement while helping residents remain active participants in their community.

Through wellness initiatives, educational opportunities, and community-building activities, residents can access resources that support independence, healthy lifestyles, and overall well-being. By bringing services and programs directly into the housing community, ESWA helps residents stay connected, informed, and engaged in the activities that matter most to them.
