At Empowering Support for Wellness & Aging (ESWA), our Nutrition Program provides much more than a meal. Through home-delivered meals, community dining opportunities, nutrition education, and wellness support, we help older adults maintain their health, independence, and quality of life. Good nutrition plays an important role in healthy aging, but so does social connections, regular contact, and access to supportive services.

For many older adults, ESWA’s Nutrition Program serves as an important link to the community. Whether through a friendly visit from a Meals on Wheels driver, a shared meal at a dining site, or connections to additional resources, our services help reduce social isolation and ensure individuals remain connected to the support they need.

ESWA’s Nutrition Program includes Meals on Wheels, Congregate Dining, Health-Related Social Needs (HRSN) support, nutrition counseling, seasonal farmers’ market opportunities, and monthly menus. Together, these services help older adults remain healthy, engaged, and independent in their homes and communities.

Meals on Wheels

More Than a Meal

Meals on Wheels provides nutritious, home-delivered meals to eligible older adults who may have difficulty preparing meals or leaving their home regularly. While nutrition is at the heart of the program, Meals on Wheels also serves as an important wellness and safety check for many participants.

For some individuals, a Meals on Wheels driver may be the only person they see that day. Drivers provide a friendly face, a brief check-in, and an opportunity to ensure participants are safe and well. These regular visits help reduce social isolation, promote wellbeing, and identify concerns that may require additional support. In many cases, the program provides peace of mind for both participants and their loved ones, knowing someone is checking in on a regular basis.

Meals are available in a variety of formats, including hot meals, cultural hot meals, frozen meals (including vegetarian), cold supper sandwiches, and therapeutic frozen meals (cardiac, diabetic, renal, low lactose, soft and pureed options). Kosher hot meals are also available for residents of the city of Worcester. These specialized meal options, tailored to individual needs, help older adults maintain their health and independence while continuing to live safely at home.

ESWA’s Congregate Dining Program offers older adults the opportunity to enjoy nutritious meals in a welcoming community setting. Located in senior centers, housing sites, and community locations throughout the service area, congregate dining provides not only healthy meals but also opportunities to socialize, build friendships, and stay connected to the community.

By bringing people together around a shared meal, the program helps reduce isolation, encourage engagement, and foster meaningful connections that support overall wellbeing.

Addressing Barriers to Better Health

Health is influenced by more than medical care alone. ESWA’s Health-Related Social Needs (HRSN) services help identify and address challenges that may affect an individual’s wellbeing, such as food insecurity, access to resources, transportation barriers, and other social factors that impact health and quality of life.

Eligibility for HRSN services is determined through an individual’s Accountable Care Organization (ACO), which assesses qualifying health-related social needs and authorizes access to available supports. By connecting eligible individuals to services and resources, ESWA helps remove barriers to healthy aging and promotes greater independence, stability, and overall wellbeing.

Personalized Guidance from a Registered Nutritionist

Nutritional needs often change with age, health conditions, and lifestyle factors. ESWA’s nutrition professionals provide education, guidance, and individualized support to help older adults make informed choices about their nutrition and overall health.

Whether managing a chronic condition, addressing dietary concerns, or simply looking for ways to eat healthier, participants can receive personalized recommendations tailored to their unique needs and goals.

Fresh, Local Produce for Healthy Living

During the growing season, ESWA helps connect eligible older adults with farmers market opportunities that increase access to fresh fruits, vegetables, and locally grown foods. These seasonal programs support healthy eating while helping participants stretch their food budgets and enjoy the benefits of fresh, nutritious produce.

By encouraging access to local food resources, the Farmers Market Program supports both healthy lifestyles and community engagement.
