Personal Care Management

At Empowering Support for Wellness & Aging (ESWA), our Personal Care Management (PCM) Program helps eligible individuals with disabilities maintain independence by giving them greater control over their personal care services. Funded through MassHealth, the program allows participants to hire, train, and supervise their own Personal Care Attendants (PCAs), ensuring care is provided by individuals they know and trust.

PCM places the participant at the center of implementing their care plan. Individuals direct their own care and determine how their approved personal care hours are used to meet their daily needs. The program is designed to promote choice, flexibility, and independence while ensuring participants receive the support necessary to live safely in their homes and communities.

ESWA’s Functional Skills Trainers and nurses work closely with participants throughout the process. They help determine initial eligibility, explain program requirements and responsibilities, assess care needs, collaborate with healthcare providers, and develop service plans for MassHealth review. They can also provide guidance and resources to help participants recruit and hire Personal Care Attendants when needed.

Once enrolled, participants maintain control over their personal care services while receiving ongoing support and guidance from ESWA. Our team helps individuals navigate the program, understand responsibilities, and access resources that contribute to successful independent living.
