Behavioral Health Support That Meets You Where You Are

Mental and behavioral health are important parts of overall wellbeing at every age. At Empowering Support for Wellness & Aging (ESWA), the Behavioral Health Outreach for Aging Populations (BHOAP) program provides older adults with short-term behavioral health support designed to help them navigate life’s challenges, strengthen coping skills, and improve overall quality of life. The program offers compassionate, person-centered services for adults age 60 and older in a setting where they feel most comfortable.

Through BHOAP, behavioral health clinicians provide assessments, individualized counseling, care planning, care coordination, and referrals based on each person’s unique needs and goals. Services are offered at no cost to participants and may be provided in the home, through telehealth, or in another community setting chosen by the individual.

Behavioral health challenges can affect anyone and may arise following life transitions, grief and loss, social isolation, health concerns, caregiving responsibilities, housing instability, or other significant life events. ESWA’s BHOAP team provides support in a welcoming, respectful environment while helping individuals connect with additional community resources and services when needed.

Because BHOAP is designed as a short-term intervention program, clinicians also help participants transition to appropriate long-term behavioral health services if ongoing support is needed. Through referrals, resource connections, and warm handoffs, ESWA helps ensure continuity of care beyond a participant’s involvement in the program.

Whether someone is experiencing emotional challenges, increased stress, social isolation, or changes affecting their daily life, BHOAP provides support, guidance, and connections to resources that help older adults remain healthy, engaged, and connected to their communities.
