Senior Care Options

At Empowering Support for Wellness & Aging (ESWA), we know that navigating healthcare and long-term support services can be challenging. The Senior Care Options (SCO) program brings Medicare, MassHealth, and community-based services together through one coordinated system of care, helping eligible older adults access the support they need to remain healthy, independent, and engaged in their communities.

SCO is designed for adults age 65 and older who qualify for both Medicare and MassHealth (Standard). Through this program, participants receive comprehensive healthcare coverage along with care coordination and access to services that support their overall wellbeing. The program’s goal is simple: help individuals remain safely in their homes and communities whenever possible while receiving the care they need.

As part of the SCO program, ESWA partners with participating health plans to provide community-based support through experienced professionals known as Geriatric Support Services Coordinators (GSSCs). Working closely with participants, family members, healthcare providers, and community partners, GSSCs help assess needs, coordinate services, develop personalized care plans, and connect individuals with resources that support their health, independence, and quality of life.

Using a team-based approach, SCO helps ensure that medical care, behavioral health services, long-term supports, and community resources work together to meet each person’s unique needs. This coordinated model gives participants and their families added peace of mind, knowing they have a dedicated team focused on helping them achieve their health and aging goals.

One Care Health Insurance

At Empowering Support for Wellness & Aging (ESWA), we understand that managing healthcare, long-term supports, and everyday needs can be complex. One Care is a health insurance program designed to simplify that experience by bringing Medicare, MassHealth, and additional support services together under one coordinated plan. The program helps eligible adults access the care, resources, and services they need to live as independently and successfully as possible.

One Care serves adults ages 21 to 64 who qualify for both Medicare and MassHealth (Standard or MassHealth CommonHealth). Participants receive access to a full range of covered medical, behavioral health, prescription, and long-term support services through a single plan and a coordinated approach to care. The goal is to ensure that healthcare and community-based services work together to support each person’s unique needs, preferences, and goals.

A key feature of One Care is the Personalized Care Team, which places the participant at the center of the planning process. Team members may include healthcare providers, care coordinators, family members, caregivers, and other support professionals who work together to develop an individualized care plan. This collaborative approach helps participants navigate services, access resources, and make informed decisions about their health and wellbeing.

Through One Care, participants may receive enhanced benefits and supports beyond traditional healthcare coverage, including long-term services and supports, community-based services, behavioral health resources, and care coordination that promotes independence and quality of life.
